Insurance Related FAQs
If you would like to confirm our in-network status with your insurance company, please note that as of May 1st, 2023, we are now listed in provider directories under our parent company name, “IPTA Clinical, LLC“. Our physical therapists, Dr. James Pumarada, Dr. Asha Pumarada, and Humberto Colmenares, can also be searched by name in many directories as well.
If you are a current and active patient with Complete Physical Rehabilitation, there is nothing that you need to do right now!
Just keep attending your scheduled PT visits according to the care plan put in place by your physical therapist. Our front office staff will contact and help guide you if there is anything that will be needed in order to continue with physical therapy.

All in-network level coverages will immediately take effect for all patients on May 1st, 2023.
Our office falls under the “Specialist” category for most insurance plans. Copays and any other financial responsibilities are normally printed directly on your insurance card.
Remember, it is our policy is to verify all insurance information when insurance will be used as the primary payment method (see FAQ page for more info).
Dr. Carla Lescano will still continue to work out of both the Elizabeth and Jersey City, NJ clinics of Complete Physical Rehabilitation. All scheduling will continue as normal, either by contacting Dr. Carla directly or speaking with either the (908) 944-9320 (Elizabeth) or (551) 236-5455 (Jersey City) clinics.
Please note that insurance claims for acupuncture will continue to be handled separately through Balance By Acupuncture.
